If you want to hire us to represent you, you will need to sign and return our Contingent Fee Agreement for Legal Services. In addition to a signed Contingent Fee Agreement, we will need you to provide us with copies of the following documents to enable us to evaluate, prepare and file your case:
(i) Your lease;
(ii) Any division orders signed by you;
(iii) Royalty statements or royalty payment summaries for the last four years (or as long as you have been receiving royalties) from Chesapeake and each other gas company from which you have received royalties;
(iv) Form 1099s received from Chesapeake and any other gas companies from which you receive royalties, as filed with your tax returns, for any royalties received; and
(v) Any written correspondence (emails or letters) with Chesapeake or other gas companies.

If you are a member of the Settlement Class in Demchak Partners, you also still will need to opt-out of the Settlement Class in order for us to represent you, by completing and mailing your opt-out request no later than the DECEMBER 17 deadlines established by the Court in that case. Information on how to opt-out of the Settlement Class in Demchak Partners, and a sample form of opt-out request, is available here.
If you don't have, or can't locate, copies of your royalty statements or summaries, don't worry. Please just get us what you have readily available, and we will help you gather the rest. You should be able to obtain some if not all of the royalty payment information online from Chesapeake. If you already have an online account with Chesapeake, you can access your payment information directly and then forward it to us, or provide us with your login and password to enable us to obtain the information for you. If you do not have an online account, you can call Chesapeake at 877-245-1427, and ask to set up an online account. Once your account is in place, you can then access your payment information directly, or provide us with your login and password so that we can take care of it for you. Important: Please do not request any additional documents other than royalty payment information during your call.
IMPORTANT: Because of the high volume of requests for representation we are receiving, and the upcoming holdidays, if you would like to retain us to file claims as part of one of our 2015 group actions, we will need to receive your signed contingent fee agreement, and copies of your lease and other necessary documents, by no later than DECEMBER 14.
