how do i sign up?
If you are interested in joining the group action, and retaining us to represent you, please
(1) sign and return our Contingent Fee Agreement for Legal Services, a copy of which is available by filling in the contact information and then following the links here, or by calling or e-mailing us and requesting a copy, and
(2) provide us with a copies of the following documents:
(a) your Lease;
(b) your Royalty Statements or check stub royalty payment summaries for the last four years
(or as many years as you have available) from all of the gas companies that paid royalties to you;
(c) copies of any Form 1099s that you received and filed with your tax returns showing royalties received; and
(d) any and all letters or other correspondence (emails) that you received from or sent to Chesapeake or any of the
other gas companies about your royalties.
IMPORTANT: Because of the high volume of requests for representation we are receiving, and the upcoming holdidays, if you would like to retain us to file claims as part of one of our 2015 group actions, we will need to receive your signed contingent fee agreement, and copies of your lease and other necessary documents, by no later than DECEMBER 14. If you are a member of the Settlement Class in Demchak Partners, you also still will need to complete and mail your opt-out request no later than DECEMBER 17. Information on how to opt-out of the Settlement Class in Demchak Partners is available here.
If you don't have or can't locate copies of all of the documents requested, don't worry, and don't let that delay you from signing up now. Send us what you have, and we can work with you to gather the rest later.
We would prefer if you did not send us your only copies of any documents, but if it is not convenient or feasible for you to make copies or scan your statements yourself, that's OK; we would be happy to arrange to scan your documents and return them to you. Just drop off or send us your documents as indicated below.
You can submit you signed Contingent Fee Agreement, lease and related royalty documents to us by Hand Delivery, Regular Mail, Overnight Delivery, E-mail or Fax – whichever method is most convenient and comfortable for you – to any member of our litigation team, as follows:
